Serving Law Enforcement, EMS, Firefighters, Dispatchers, and Military Personnel

Every day, first responders run toward danger—rushing into burning buildings, responding to violent crises, treating the wounded, and standing strong in the face of trauma.

But who answers the call for you?

At River’s Bend, we proudly introduce our Frontline Resilience Program, a specialized mental health and substance use treatment track designed exclusively for:

Active-duty and retired police officers

Firefighters

EMTs and paramedics

911 dispatchers and communication specialists

Military members, reservists, and veterans

For Many of Us, This Commitment Is Personal

Our team’s connection to this work runs deep.

Bruce Goldberg, River’s Bend Co-Owner, comes from a family of first responders. His grandfather served with the NYPD, and that legacy of service continues to shape our mission.

Cheryl Narduzzi, Business Development Liaison, is married to a police officer and brings firsthand insight into the needs of first responder families.

Logan Cook, LLMSW, was inspired by his father’s proud service and deep commitment to community. Logan recently completed a PsychArmor certification course, a trusted, expert-led training designed to help professionals better understand and support service members, veterans, and their families.

Jessica Hillen, LMSW, Clinical Director, is married to a police officer and understands the complex balance between duty, trauma, and family life.

Mark Kauffman, LPC, CPSS, honors his late father, a retired police officer, through his dedication to helping first responders and veterans recover and thrive.

These personal experiences, and continued education in responder and military wellness, drive our compassion, authenticity, and clinical excellence.

Ongoing Education & Partnership: Learning from Leaders in Trauma-Informed Care

At River’s Bend, we believe the best care comes from continuous learning.

Recently, Mark Kauffman, Jessica Hillen, and Logan Cook visited Sierra Tucson’s Red White and Blue Program, which specializes in treatment tracks for military personnel, veterans, retirees, and first responders.

This collaboration allowed our clinicians to gain deeper insight into:

The challenges faced by service members transitioning from active duty

Advanced therapeutic models for PTSD, addiction, and moral injury

Integrative approaches to recovery that combine clinical, holistic, and community support

Why First Responders Need Specialized Mental Health & Substance Use Care

Being a first responder isn’t just a job; it’s a calling. But that calling comes with extraordinary demands.

Shift work, trauma exposure, and the pressure to remain “unbreakable” can lead to anxiety, depression, or substance use. Many turn to alcohol or drugs not from weakness, but from a buildup of unprocessed stress.

Our Frontline Resilience Program integrates co-occurring mental health and substance use treatment for true, whole-person recovery.

Comprehensive, Culture-Informed Care for First Responders

1. Clinical Assessment for the Right Level of Care

Our licensed clinicians understand responder culture and confidentiality needs. Whether you’re experiencing PTSD, burnout, depression, or addiction, we’ll help you find the right level of care.
🔹 Why it matters: Trust begins with cultural competence and confidentiality.

2. Individual Therapy

Confidential one-on-one sessions using CBT, ACT, and trauma-focused therapy to reduce hypervigilance, anxiety, and emotional fatigue.
🔹 Why it matters: Private, targeted therapy for responders, by clinicians who understand your world.

3. Group Therapy

Peer-based sessions help responders connect and heal through shared experience.
🔹 Why it matters: Connection combats isolation, the root of burnout and despair.

4. EMDR Trauma Therapy

Eye Movement Desensitization and Reprocessing (EMDR) helps process critical incident trauma safely and effectively.
🔹 Why it matters: You don’t have to relive the pain to find relief.

5. Medication Management

When indicated, our psychiatric team supports stability and function through individualized care.
🔹 Why it matters: The goal is restoration, not sedation.

6. Mental Health Intensive Outpatient Program (IOP)

Three structured therapy sessions per week (in-person or virtual) for 6–8 weeks.
🔹 Why it matters: Balancing recovery with real life.

7. Substance Use Disorder (SUD) IOP

An integrated track that blends psychoeducation, relapse prevention, and therapist-led peer processing.
🔹 Why it matters: Recovery without shame, within a structure that works for first responders.

Expanded Expertise In Clinical Training

Three River’s Bend therapists recently explored the Sierra Tucson “Red White and Blue” program, a specialized training for clinicians serving law enforcement and first responders.

This advanced education enhances our clinicians’ ability to:

Recognize responder-specific trauma responses

Navigate departmental culture and confidentiality concerns

Deliver therapy that respects the realities of the job while promoting long-term wellness

Our participation in this program underscores River’s Bend’s mission to provide culturally competent, clinically advanced care for those who serve our communities.

New Resource: Family Action Plan for First Responder Families

Healing doesn’t happen in isolation; it happens in relationships.

River’s Bend has developed a new, interactive Family Action Plan for First Responder Families, a proactive, evidence-informed guide to help families build resilience together and prevent burnout or crisis before it begins.

Inside you’ll find:

A Family Resilience Checklist

Tools to support open communication about stress and trauma

Step-by-step guidance for developing a Family Crisis & Wellness Plan

Local Michigan and national mental health resources

Use it as a standalone resource or bring it into therapy sessions for personalized application and family goal setting.

What Makes River’s Bend Different

Clinicians trained in first responder and veteran culture

“Back the Blue” certification & PsychArmor veteran-care training

Collaborative insights from Sierra Tucson’s trauma recovery programs

Co-occurring care for mental health and addiction

Flexible scheduling for shift work

Confidential, trauma-informed treatment spaces

In-person and virtual care options

A Message from Bruce Goldberg, Co-Owner: Why Asking for Help Is Leadership

“I was trained to help others. I never learned how to ask for help myself.”

That’s what we hear most often from first responders. But asking for help isn’t weakness, it’s leadership.

We’ve walked alongside countless first responders and veterans rediscovering their purpose, rebuilding relationships, and reclaiming peace. Whether you’re struggling with trauma, anxiety, or substance use, we’ll meet you with compassion, respect, and expertise.

A Word from the Front Lines

“First responders face hundreds of traumatic events in a single career—far more than most will experience in a lifetime.

Take the First Step Toward Resilience

If you’re a first responder, EAP professional, or department leader, River’s Bend is here to help.

Why Treating Mental Health and Substance Use Together is Essential.

Mental health and substance use disorders are deeply interconnected. When someone struggles with both conditions—a situation known as a co-occurring disorder or dual diagnosis—treating only one issue while neglecting the other often leads to ineffective treatment and a higher risk of relapse. Integrated care, treating both conditions simultaneously, offers a comprehensive and evidence-based approach to sustained recovery and long-term mental wellness.

At River’s Bend, we understand that no two individuals experience addiction and mental health challenges in the same way. Our integrated treatment approach ensures that each client receives personalized care that addresses the full spectrum of their needs, fostering lasting stability and healing.

Understanding Co-Occurring Disorders

Co-occurring disorders are common. According to the Substance Abuse and Mental Health Services Administration (SAMHSA), nearly 50% of individuals who experience a substance use disorder will also face a mental health condition at some point in their lives. Frequently occurring combinations include:

Depression & Alcohol Use Disorder – Individuals struggling with depression may use alcohol to cope with persistent sadness or lack of motivation, yet prolonged alcohol use can worsen depressive symptoms.

Anxiety & Stimulant Use – Stimulants like cocaine or amphetamines can temporarily relieve feelings of anxiety or stress, but over time, they heighten paranoia, panic attacks, and emotional instability.

Bipolar Disorder & Substance Abuse – The extreme highs and lows of bipolar disorder often lead individuals to self-medicate with drugs or alcohol, which disrupts mood stabilization and treatment progress.

PTSD & Opioid Dependence – Trauma survivors may turn to opioids to numb intrusive memories and emotional pain, leading to dependency and worsened PTSD symptoms.

These overlapping conditions create a complex cycle where substance use fuels mental health symptoms, and vice versa. Without integrated treatment, individuals often experience misdiagnoses, incomplete recovery, and frequent relapses.

Why Integrated Care is the Key to Long-Term Recovery

Historically, mental health and addiction treatment were addressed separately, often leading to gaps in care and poor outcomes. At River’s Bend, our mission-based, integrated approach treats co-occurring disorders as interconnected conditions rather than isolated issues. This method offers several crucial benefits:

1. A Unified Treatment Plan for the Whole Person

Rather than bouncing between separate mental health and addiction programs, our integrated care offers a single, coordinated treatment plan that addresses both issues simultaneously. Our approach includes:

Medical and Psychiatric Support – Clients receive comprehensive evaluations to determine the best course of treatment, including medication-assisted treatment (MAT) when necessary.

Therapeutic Interventions – Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed therapy help clients develop healthy coping mechanisms.

Group & Family Therapy – Healing doesn’t happen in isolation. Support groups and family therapy reinforce recovery and rebuild relationships.

2. Lower Risk of Relapse & Better Long-Term Outcomes

When only one disorder is treated, underlying issues often resurface, leading to relapse. Integrated care ensures that clients:

Develop healthier coping skills instead of turning to substances.

Receive consistent support through both mental health and addiction treatment teams.

Are less likely to experience treatment drop-out or disengagement.

3. A Seamless Step-Down Process from Intensive Care

At River’s Bend, we offer a structured continuum of care, allowing clients to transition from higher to lower levels of treatment as they progress. Our treatment options include:

Partial Hospitalization Program (PHP) – Ideal for individuals who need structured, full-day support without 24/7 hospitalization.

Intensive Outpatient Program (IOP) – A flexible option that provides multiple therapy sessions per week while allowing clients to return to daily responsibilities.

Outpatient & Continuing Care – Ongoing therapy and relapse prevention planning help clients maintain stability after completing intensive treatment.

Learn more about how PHP and IOP work together to ensure long-term recovery and wellness.

Choosing the Right Treatment: How to Get Started

If you’re a healthcare professional or referral partner, identifying co-occurring disorders early and referring individuals to integrated care is critical. Signs that a client may benefit from dual diagnosis treatment include:

A history of relapse despite previous treatment

Frequent hospitalizations for mental health crises or overdoses

Increased substance use following traumatic events

Difficulty maintaining employment, relationships, or self-care routines

At River’s Bend, we are committed to helping clients break free from the cycle of addiction and mental health challenges. Our team of experienced clinicians, therapists, and addiction specialists work together to provide personalized, compassionate care that leads to lasting change.

Take the Next Step Toward Lasting Recovery

If you or someone you know is struggling with co-occurring mental health and substance use disorders, help is available.

Contact River’s Bend today to schedule a consultation.

Every individual’s path to healing is unique, requiring different levels of support and care along the way. Two critical components of a comprehensive recovery plan are Partial Hospitalization Programs (PHP) and Intensive Outpatient Programs (IOP). While each serves a distinct purpose, they often work best when combined, ensuring a seamless transition from structured care to long-term independence.

At River’s Bend, we understand that sustainable recovery requires a balance of support, structure, and personal responsibility. This guide will help you understand the key differences between PHP and IOP, how they complement each other, and when one might be the right choice for you or a loved one.

What Is a Partial Hospitalization Program (PHP)?

A Partial Hospitalization Program (PHP) is one of the highest levels of outpatient care available. It provides intensive therapy while allowing clients to return home or to a sober living environment in the evenings.

Key Features of PHP:

Structured day-long therapy sessions, 6 hours per day, for 10–15 weekdays depending on individual needs.

Access to medical and psychiatric support.

Individual and group therapy.

Holistic approaches such as mindfulness, nutrition, and stress management.

A step-down alternative to inpatient treatment for those who still need intensive care but don’t require 24/7 supervision.

Typical PHP Day Schedule

Morning welcome and check-in group

Didactic Session: education

Group Process: personal experience of a topic

Lunch and Break

Skills Group

Skills Activation Group

Check out group and goal-setting

Who Benefits Most from PHP?

Individuals transitioning from residential/inpatient rehab

Those needing medical and psychiatric oversight while stabilizing

Clients requiring intensive therapy but with a supportive home environment

What Is an Intensive Outpatient Program (IOP)?

An Intensive Outpatient Program (IOP) for mental health or substance abuse provides structured support for individuals while allowing them to maintain work, school, and family commitments. It is less time-intensive than PHP but offers comprehensive therapy and relapse prevention tools. The program runs for an average of 6–8 weeks but can be individualized depending on a client’s needs. Clients can choose from daytime, afternoon, or evening, virtual or in-person. During the session, various topics are presented and discussed, allowing for group discussion facilitated by a certified therapist. At River’s Bend, IOPs are offered in specialized tracks for men, women, trauma, and grief to ensure more focused, relevant, and supportive care tailored to each client’s needs.

Key Features of IOP:

Therapy sessions are 3 times a week, for 3 hours per session

Focus on relapse prevention, coping strategies, and life skills

Encourages reintegration into daily life while maintaining treatment support

Who Benefits Most from IOP?

Individuals transitioning from PHP or inpatient rehab

Those who need continued therapy but with more flexibility

Clients who have established some stability but still require accountability and structure

How PHP and IOP Work Together for Long-Term Recovery

Many clients start in PHP and transition to IOP as they gain stability and independence. This gradual step-down approach ensures that they receive the support they need at each phase of recovery without feeling overwhelmed or isolated.

Benefits of Combining PHP & IOP:

Smooth Transition: Moving from PHP to IOP helps maintain continuity of care without abrupt changes.

Personalized Support: Treatment plans are tailored, allowing clients to receive appropriate levels of care as they progress.

Preventing Relapse: A structured step-down process significantly reduces the risk of relapse by providing ongoing support.

Encouraging Independence: While PHP offers intensive guidance, IOP encourages clients to apply recovery skills in real-life situations.

Many individuals seeking PHP or IOP treatment may also struggle with co-occurring mental health and substance use disorders, making integrated care essential for long-term success. At River’s Bend, we emphasize treating both addiction and mental health together to ensure a comprehensive, evidence-based recovery plan. Learn more about why treating mental health and substance use together is essential.

PHP vs. IOP?

It’s important to select the right level of care based on your needs, progress, and support system.
