A Clinical Guide for Providers Navigating Co-Occurring Disorders

Many behavioral health professionals face this challenge:
You’ve built trust with a patient struggling with addiction. They’re engaged, showing some progress, yet something isn’t clicking. Emotional volatility, inconsistent follow-through, frequent relapse, or unresolved trauma keep pulling them back.

The answer, more often than not, is clinical: your patient may be experiencing a co-occurring disorder, and without integrated care, their addiction treatment may not succeed long-term.

The Clinical Reality: Addiction and Mental Health Rarely Exist Alone

Research shows that more than 50% of individuals with a substance use disorder also meet the criteria for at least one mental health diagnosis, such as depression, anxiety, PTSD, or bipolar disorder.¹

According to the Substance Abuse and Mental Health Services Administration (SAMHSA), co-occurring disorders (CODs) are underdiagnosed and undertreated in traditional outpatient settings, often due to fragmented care or provider hesitation to refer for dual-diagnosis treatment.²

Why It Matters

If addiction treatment is progressing, but mental health remains unaddressed, symptoms like emotional dysregulation, shame, trauma responses, or social withdrawal can fuel relapse or treatment resistance.³

When Standard Outpatient Isn’t Enough

As a provider, you may notice a few signs that a patient’s addiction treatment needs more than what you can offer in weekly therapy or standard outpatient care.

Clinical Red Flags of a Co-Occurring Disorder:

• Repeated relapse despite motivation or engagement
• Inability to regulate emotions or mood between sessions
• Panic attacks, depressive spirals, or suicidal ideation in early recovery
• Trauma symptoms surfacing during sobriety
• Medication compliance issues with underlying mental health conditions
• Substance use tied directly to mood management (e.g., using to sleep, numb, or “feel normal”)

These aren’t signs of noncompliance or failure. They’re signs the client needs structured, integrated treatment for both addiction and mental health in one setting—not piecemeal care across multiple providers.

The Evidence for Dual-Diagnosis Treatment

Studies consistently show that integrated treatment for co-occurring disorders leads to:⁴

• Lower relapse rates
• Improved psychiatric symptom management
• Higher treatment retention
• Better long-term functioning and recovery outcomes

When to Refer to IOP vs. PHP

We understand how hard it is to make a referral when you don’t want your client to feel like you’re “passing them off.” That’s why we partner with you, not replace you.

Here’s a clinical breakdown of when a referral may be appropriate:

Refer to Mental Health or Substance Use Intensive Outpatient Program (IOP) When:

• Client is actively using or newly sober but safe to remain in the community
• Weekly therapy is not sufficient for symptom stabilization
• Client is experiencing mood instability, anxiety, trauma, or unresolved grief interfering with addiction recovery
• Client is asking for more support, more structure, or more tools

River’s Bend IOPs run three times per week (day, evening, and virtual options), offering nine hours of structured group and individual therapy, ideal for clients who need support while maintaining work or family responsibilities.

Refer to Partial Hospitalization Program (PHP) When:

• Client has recently discharged from inpatient detox or hospitalization and needs step-down care
• Client’s symptoms (emotional, psychiatric, or addiction-related) interfere with basic functioning
• There’s risk of relapse or deterioration without daily support
• You’re concerned about safety or rapid decompensation, but 24/7 hospitalization isn’t clinically required

Our PHP program offers full-day treatment (M–F, 9:00 AM to 3:30 PM) and close coordination with outside providers to ensure safety and continuity.

Keep the Relationship. Share the Care.

You’ve built trust with your client, and referring them to River’s Bend doesn’t end that relationship. We encourage collaboration with outside providers, frequent communication (with consent), and flexible re-engagement once the client is ready to return to standard outpatient.

We often say to referring therapists: “You don’t have to give them up. You just don’t have to hold the full clinical weight alone.”

Here’s how River’s Bend evolved into one of the area’s most respected IOP and PHP behavioral health providers.

1995–2006: Building a Foundation in Outpatient Behavioral Health

River’s Bend opened in 1995 with a clear mission: to provide clinically rigorous, client-centered outpatient mental health and addiction treatment in Metro Detroit.

By 1998, Stephanie Howey joined the team, helping establish a culture rooted in collaboration, professional growth, and personalized care. Her leadership laid the groundwork for internal development and long-term clinical excellence.

In 2000, Bruce Goldberg expanded services by launching our Substance Use Disorder Intensive Outpatient Program (IOP), offering structured recovery support for individuals seeking outpatient addiction treatment in Oakland County.

2007–2016: Expanding Access to Integrated Care

In 2007, River’s Bend entered a new chapter when Bruce Goldberg and Stephanie Howey became owners. At that time, the clinic had four therapists and a primary focus on substance use treatment. However, community demand quickly revealed the need for integrated mental health and co-occurring disorder services.

By 2009, River’s Bend became an approved provider for Michigan’s Health Professional Recovery Program (HPRP), delivering structured, evidence-based outpatient care to licensed professionals navigating recovery.

This period marked our transition into a comprehensive behavioral health provider serving Metro Detroit.

2017–2023: Responding to the Opioid Crisis & Post-Pandemic Mental Health Needs

As the opioid epidemic intensified, River’s Bend strengthened partnerships with hospitals, physicians, and community providers to create rapid referral systems and coordinated step-down care.

Following the pandemic, demand for higher levels of care surged. In response, we launched:

• A Mental Health Intensive Outpatient Program (IOP) for depression, anxiety, and trauma
• A reopened Adolescent IOP in West Bloomfield
• A Partial Hospitalization Program (PHP) for clients requiring stabilization without inpatient hospitalization

In 2023, we expanded further by opening our West Bloomfield behavioral health clinic, increasing access to structured group therapy and specialty programming throughout Oakland County.

2023–2025: Specialized IOP Tracks for Diverse Clinical Needs

To meet growing demand, River’s Bend introduced 15+ specialized IOP tracks, including programming tailored to:

• Trauma survivors
• Adolescents and teens
• Working professionals
• Women and men’s groups
• LGBTQIA+ individuals
• Clients with co-occurring substance use disorders

In 2025, we launched a dedicated First Responder IOP, providing trauma-informed care for police officers, firefighters, EMTs, and frontline professionals experiencing occupational stress and secondary trauma.

This expansion strengthened our position as a leader in specialized outpatient mental health and addiction treatment in Southeast Michigan.

2026 and Beyond: Advancing Trauma-Informed Treatment

Looking ahead, River’s Bend continues investing in clinical excellence. In 2026, we are expanding advanced trauma training, including broader clinician certification in EMDR (Eye Movement Desensitization and Reprocessing).

By integrating cutting-edge trauma therapies into our IOP and PHP programs, we remain committed to delivering evidence-based, outcomes-driven care across Metro Detroit.
