From Intern to Clinical Director

At River’s Bend PC, healing isn’t just clinical, it’s deeply human. And few people embody that truth more than Amy Fresch, MA, LPC. As Clinical Director, Amy leads with a rare blend of therapeutic skill, personal wisdom, and unwavering compassion. Her journey from intern to clinical leader is more than a success story, it’s a blueprint for how lived experience can shape transformative care.

A Leader Who Understands Because She’s Lived It

Amy’s life has been shaped by the same challenges many of her clients face. Growing up in a family impacted by addiction and trauma, and standing alongside her husband in long-term recovery, Amy brings an unshakable empathy to every therapeutic space she enters.

Her message is clear: “You are not broken, you are becoming.”

This perspective informs everything she does:

Building a trauma-informed culture across River’s Bend’s Troy and West Bloomfield clinics

Supporting clients through mental health and substance use recovery with authentic, evidence-based care

Training clinicians to lead with heart, listen deeply, and create space for all parts of the healing journey

Innovating Intensive Outpatient Therapy

One of Amy’s signature contributions is her development of River’s Bend’s 10-week, 30-lecture Mental Health IOP curriculum. Drawing on modalities like CBT, DBT, and motivational interviewing, she crafted a program that is both structured and soul-centered, offering real tools for recovery while meeting clients exactly where they are.

Her specialties include:

Group therapy for clients in early recovery

Co-occurring disorders (mental health + substance use)

Curriculum development that turns theory into life-changing practice

Amy’s sessions are known for their depth. Clients often say, “She saw me when no one else did.” And that’s exactly her goal.

Calm in the Storm: When Clinical Skill Meets Human Presence

Amy is more than a curriculum builder, she’s a clinician who shows up in the hardest moments. In one virtual group, when a client disclosed a suicide attempt mid-session, Amy calmly de-escalated, contacted emergency services, and remained online with the client until help arrived. Later, she welcomed him back into care.

That moment didn’t define her, it affirmed her: Amy is the lifeline people need when everything feels like it’s falling apart.

Mentoring the Next Generation of Heart-Led Therapists

Today, Amy oversees and mentors River’s Bend’s growing team of clinicians. She teaches therapists to:

Watch for the quiet ones

Stay grounded in crisis

Challenge imposter syndrome

Lead with authenticity and empathy

She often shares her own journey of returning to school later in life, confronting the internal voice that said, “You’re not smart enough.” Her leadership reminds both clients and clinicians that healing isn’t linear, and it’s never too late to begin again.

Specializations

Co-occurring disorder treatment (mental health + substance use)

Group therapy facilitation for IOP

Clinical curriculum development

Trauma-informed crisis intervention

Recovery support for individuals and families

Holding Hope in Every Session

Amy Fresch doesn’t just lead a clinical department, she leads a movement toward deeper, more personal mental health care. Whether guiding someone through relapse, coaching a therapist through burnout, or offering a message of hope to someone in early recovery, Amy brings her full self, present, honest, and hopeful.

From Recovery to Leadership

In the world of behavioral healthcare, few leaders blend clinical excellence with personal courage as seamlessly as Amy Buchanan, MA. As Practice Manager of River’s Bend PC, Amy is the operational force behind the scenes—and the compassionate voice many clients first hear when they reach out for help.

Her impact stretches far beyond schedules and spreadsheets. A woman in long-term recovery herself, Amy brings a rare and powerful combination of administrative expertise and lived experience to River’s Bend’s outpatient clinics in Troy and West Bloomfield.

Leadership with Lived Experience

Amy’s path to leadership is rooted in resilience. She first shared her recovery story just weeks after treatment at a Unite to Face Addiction–Michigan event. That moment—met with a standing ovation—launched her advocacy career, leading her to:

Speak at the Michigan Capitol on addiction recovery and stigma

Mentor individuals and families navigating early recovery

Appear in the award-winning documentary My Secret is My Story

Partner with organizations like FAN (Face Addiction Now) and The Amber Reineck House

Her message is always grounded in hope and humility: “My story doesn’t define me—but it reminds me who I’m here to help.”

Creating Safe Space for Recovery—One Call at a Time

In her role at River’s Bend, Amy is often the first point of contact for people in crisis. Whether she’s answering the phones, supporting staff, or comforting a parent, she meets each moment with deep empathy and zero judgment.

Amy’s work includes:

Training front desk and support staff in trauma-informed care

Providing peer-informed support to clients and caregivers

Managing daily operations across both River’s Bend locations

Coordinating outreach and partnerships with community groups

She doesn’t carry the title of peer recovery coach—but she embodies the values: dignity, presence, and nonjudgmental support. “Sometimes people just need to hear, ‘You’re not alone,’” she says. “And I can say that with my whole heart.”

Administrative Backbone. Community Voice.

With a master’s in administration and over two decades of leadership experience in school systems, nonprofits, and clinics, Amy bridges clinical care with operational excellence. Her focus is always client-centered, even when working behind the scenes.

Her community work includes:

Collaborating with school districts on youth mental health outreach

Advocating for recovery resources across Oakland and Macomb Counties

Speaking to families, students, and lawmakers about the real face of recovery

Amy balances logistics with lived empathy, demonstrating that behavioral healthcare is strongest when it includes people who’ve walked the path themselves.

Areas of Focus

Behavioral health clinic operations & systems

Trauma-informed frontline staff training

Peer-informed family and client support

Advocacy and stigma reduction speaking

Recovery outreach in schools and communities

A Leader Who Makes Recovery Personal

Amy Buchanan leads with the kind of authenticity that can’t be taught. Her journey from early recovery to administrative leadership is more than a personal milestone—it’s a guiding light for others seeking healing, hope, and connection.

A Clinical Guide for Providers Navigating Co-Occurring Disorders

Many behavioral health professionals face this challenge:
You’ve built trust with a patient struggling with addiction. They’re engaged, showing some progress, yet something isn’t clicking. Emotional volatility, inconsistent follow-through, frequent relapse, or unresolved trauma keep pulling them back.

You wonder: Is this a treatment plateau… or are we missing part of the picture?

The answer, more often than not, is clinical: your patient may be experiencing a co-occurring disorder, and without integrated care, their addiction treatment may not succeed long-term.

The Clinical Reality: Addiction and Mental Health Rarely Exist Alone

Research shows that more than 50% of individuals with a substance use disorder also meet the criteria for at least one mental health diagnosis, such as depression, anxiety, PTSD, or bipolar disorder.1

According to the Substance Abuse and Mental Health Services Administration (SAMHSA), co-occurring disorders (CODs) are underdiagnosed and undertreated in traditional outpatient settings, often due to fragmented care or provider hesitation to refer for dual-diagnosis treatment.2

Why It Matters

If addiction treatment is progressing, but mental health remains unaddressed, symptoms like emotional dysregulation, shame, trauma responses, or social withdrawal can fuel relapse or treatment resistance.3

At River’s Bend, we routinely receive referrals from therapists, counselors, and primary care providers who are seeing these patterns. Our integrated approach ensures they don’t lose the relationship they’ve worked hard to build, we simply help support it at a higher level of care.

When Standard Outpatient Isn’t Enough

As a provider, you may notice a few signs that a patient’s addiction treatment needs more than what you can offer in weekly therapy or standard outpatient care.

Clinical Red Flags of a Co-Occurring Disorder:

Repeated relapse despite motivation or engagement

Inability to regulate emotions or mood between sessions

Panic attacks, depressive spirals, or suicidal ideation in early recovery

Trauma symptoms surfacing during sobriety

Medication compliance issues with underlying mental health conditions

Substance use tied directly to mood management (e.g., using to sleep, numb, or “feel normal”)

These aren’t signs of noncompliance or failure. They’re signs the client needs structured, integrated treatment for both addiction and mental health in one setting—not piecemeal care across multiple providers.

The Evidence for Dual-Diagnosis Treatment

Studies consistently show that integrated treatment for co-occurring disorders leads to:4

Lower relapse rates

Improved psychiatric symptom management

Higher treatment retention

Better long-term functioning and recovery outcomes

Our clinicians use structured evidence-based models that treat substance use and mental health conditions simultaneously, including:

CBT and DBT for both addiction and mood regulation

Trauma-informed therapy

Group process designed to address emotional and behavioral patterns

Psychiatric support and recovery planning

When to Refer to IOP vs. PHP

We understand how hard it is to make a referral when you don’t want your client to feel like you’re “passing them off.” That’s why we partner with you, not replace you.

Here’s a clinical breakdown of when a referral may be appropriate:

Refer to Mental Health or Substance Use Intensive Outpatient Program (IOP) When:

Client is actively using or newly sober but safe to remain in the community

Weekly therapy is not sufficient for symptom stabilization

Client is experiencing mood instability, anxiety, trauma, or unresolved grief interfering with addiction recovery

Client is asking for more support, more structure, or more tools

River’s Bend IOPs run three times per week (day, evening, and virtual options), offering nine hours of structured group and individual therapy, ideal for clients who need support while maintaining work or family responsibilities.

Refer to Partial Hospitalization Program (PHP) When:

Client has recently discharged from inpatient detox or hospitalization and needs step-down care

Client’s symptoms (emotional, psychiatric, or addiction-related) interfere with basic functioning

There’s risk of relapse or deterioration without daily support

You’re concerned about safety or rapid decompensation, but 24/7 hospitalization isn’t clinically required

Our PHP program offers full-day treatment (M–F, 9:00 AM to 3:30 PM) and close coordination with outside providers to ensure safety and continuity.

Keep the Relationship. Share the Care.

You’ve built trust with your client, and referring them to River’s Bend doesn’t end that relationship.
Since opening its doors in 1995, River’s Bend has hit many milestones and has grown from a small outpatient behavioral Since 1995, River’s Bend PC has grown from a small outpatient clinic into a regional leader in mental health and substance use disorder treatment in Metro Detroit. Through compassionate care, clinical innovation, and evidence-based programming, we’ve become a trusted provider across Oakland County, Macomb County, and Southeast Michigan.

Here’s how River’s Bend evolved into one of the area’s most respected IOP and PHP behavioral health providers.

1995–2006: Building a Foundation in Outpatient Behavioral Health

River’s Bend opened in 1995 with a clear mission: to provide clinically rigorous, client-centered outpatient mental health and addiction treatment in Metro Detroit.

By 1998, Stephanie Howey joined the team, helping establish a culture rooted in collaboration, professional growth, and personalized care. Her leadership laid the groundwork for internal development and long-term clinical excellence.

In 2000, Bruce Goldberg expanded services by launching our Substance Use Disorder Intensive Outpatient Program (IOP), offering structured recovery support for individuals seeking outpatient addiction treatment in Oakland County.

2007–2016: Expanding Access to Integrated Care

In 2007, River’s Bend entered a new chapter when Bruce Goldberg and Stephanie Howey became owners. At that time, the clinic had four therapists and a primary focus on substance use treatment. However, community demand quickly revealed the need for integrated mental health and co-occurring disorder services.

By 2009, River’s Bend became an approved provider for Michigan’s Health Professional Recovery Program (HPRP), delivering structured, evidence-based outpatient care to licensed professionals navigating recovery.

This period marked our transition into a comprehensive behavioral health provider serving Metro Detroit.

2017–2023: Responding to the Opioid Crisis & Post-Pandemic Mental Health Needs

As the opioid epidemic intensified, River’s Bend strengthened partnerships with hospitals, physicians, and community providers to create rapid referral systems and coordinated step-down care.

Following the pandemic, demand for higher levels of care surged. In response, we launched:

A Mental Health Intensive Outpatient Program (IOP) for depression, anxiety, and trauma

A reopened Adolescent IOP in West Bloomfield

A Partial Hospitalization Program (PHP) for clients requiring stabilization without inpatient hospitalization

In 2023, we expanded further by opening our West Bloomfield behavioral health clinic, increasing access to structured group therapy and specialty programming throughout Oakland County.

2023–2025: Specialized IOP Tracks for Diverse Clinical Needs

To meet growing demand, River’s Bend introduced 15+ specialized IOP tracks, including programming tailored to:

Trauma survivors

Adolescents and teens

Working professionals

Women and men’s groups

LGBTQIA+ individuals

Clients with co-occurring substance use disorders

In 2025, we launched a dedicated First Responder IOP, providing trauma-informed care for police officers, firefighters, EMTs, and frontline professionals experiencing occupational stress and secondary trauma.

This expansion strengthened our position as a leader in specialized outpatient mental health and addiction treatment in Southeast Michigan.

2026 and Beyond: Advancing Trauma-Informed Treatment

Looking ahead, River’s Bend continues investing in clinical excellence. In 2026, we are expanding advanced trauma training, including broader clinician certification in EMDR (Eye Movement Desensitization and Reprocessing).

By integrating cutting-edge trauma therapies into our IOP and PHP programs, we remain committed to delivering evidence-based, outcomes-driven care across Metro Detroit.

Why River’s Bend Matters in Southeast Michigan

River’s Bend has not simply grown—we have continuously adapted to meet the evolving behavioral health needs of our community.

Our legacy reflects:

Integrated treatment for co-occurring mental health and substance use disorders

Accessible IOP and PHP programs in Troy and West Bloomfield

Strong partnerships with hospitals, therapists, and referral networks

A belief that healing requires structure, compassion, and evidence-based care
