Understanding Partial Hospitalization

When someone needs more support than weekly therapy or even an Intensive Outpatient Program can provide, a Partial Hospitalization Program, or PHP, may be the right next step. Despite the name, PHP does not mean staying overnight in a hospital. Instead, it is a structured, full-day treatment program that offers intensive care during the day while allowing clients to return home in the evening.

Who Is PHP Designed For?

PHP is designed for people who need significant support for mental health or substance use concerns but do not require 24-hour inpatient hospitalization. It can be especially helpful for individuals who are stepping down from inpatient care, coming out of a crisis period, or struggling with symptoms that are interfering with daily functioning.

What Happens in a PHP?

In a PHP, treatment is more intensive than IOP. Clients typically attend programming five days a week for several hours each day. During that time, they may participate in group therapy, individual therapy, psychiatric support, medication management, skills training, psychoeducation, and care coordination. The goal is to provide stabilization, structure, and clinical momentum while helping clients begin transitioning back into everyday life.

Mental Health and Substance Use Support

PHP can be beneficial for people experiencing depression, anxiety, trauma, emotional dysregulation, grief, relapse risk, or co-occurring mental health and substance use issues. It can also help those who feel unsafe or overwhelmed with standard outpatient care but are medically stable enough to live at home.

Why Structure Matters in PHP

One of the most important features of PHP is the level of consistency it offers. When symptoms feel intense, healing often requires more than a once-a-week conversation. PHP creates a therapeutic routine that helps reduce isolation, build coping skills, and provide close clinical monitoring. That kind of structure can be especially valuable when someone is vulnerable to setbacks, hospitalization, or relapse.

PHP vs. IOP: What Is the Difference?

Many people wonder about the difference between PHP and IOP. The simplest answer is intensity and time commitment. PHP is usually a full-day program with more frequent support, while IOP typically meets a few times a week for shorter sessions. Both are valuable, but they serve different needs at different stages of recovery.

If you are comparing these options, read What Is an Intensive Outpatient Program (IOP)? for a closer look at how IOP works.

PHP as Part of a Larger Care Plan

PHP can also serve as part of a larger continuum of care. Someone may begin in PHP for stabilization, then step down to IOP as they gain strength and confidence. This gradual transition can make recovery feel more sustainable and less overwhelming.

When Substance Use Is Part of the Concern

If your concerns involve alcohol or drug use, PHP may also be part of treatment when substance use is affecting safety, stability, or mental health. To understand how clinicians think about these concerns, visit What Qualifies as Substance Abuse? If alcohol is the main concern, Do I Have a Drinking Problem? can help you think through common warning signs.

Taking the Next Step Toward Stability

Seeking a higher level of care can feel intimidating, but it is often a sign of insight, not failure. PHP offers a bridge between crisis and stability. For people who need meaningful support without overnight hospitalization, it can be a powerful place to begin rebuilding health, hope, and daily life.

Finding the Right Level of Care

When someone is struggling with mental health symptoms or substance use, it can be hard to know what level of care is right. Weekly therapy may not feel like enough, but inpatient treatment may not be necessary. In many cases, an Intensive Outpatient Program, or IOP, offers the right balance of structure, flexibility, and support.

How an Intensive Outpatient Program Works

An Intensive Outpatient Program is a higher level of care than traditional outpatient therapy, but it does not require an overnight stay. Clients attend treatment several times a week for multiple hours per session, then return home afterward. This allows people to receive consistent clinical care while continuing to manage work, school, family, or other daily responsibilities.

Who Can Benefit From IOP?

IOP is often recommended for people who need more support than weekly counseling can provide. It can help individuals dealing with anxiety, depression, trauma, grief, mood changes, relapse risk, or substance use concerns. It may also be a strong next step after inpatient treatment, detox, residential care, or a more intensive program like Partial Hospitalization.

What Happens in IOP?

So, what happens in IOP? While every program is different, IOP typically includes a combination of group therapy, individual support, psychoeducation, coping skills training, relapse prevention, and treatment planning. Clients learn practical tools they can use in real life, not just in a therapy room. That might include strategies for managing triggers, improving communication, regulating emotions, and building healthier routines.

Practicing Recovery in Everyday Life

One of the biggest benefits of IOP is that it helps people practice recovery in the context of everyday life. Because clients go home after treatment, they can begin applying what they learn right away. Then they come back to process challenges, celebrate progress, and strengthen new skills with clinical support.

Mental Health, Substance Use, or Both

For some people, IOP focuses primarily on mental health. For others, it centers on substance use recovery. In many cases, treatment addresses both at the same time. This matters because mental health and substance use often overlap. Someone may be drinking to cope with anxiety, or using substances while also experiencing depression, trauma, or emotional distress. Treating both together can lead to better long-term outcomes.

The Value of Group Support

IOP is also helpful because it offers community. Many people feel isolated when they are struggling. Group-based care reminds clients they are not alone. It creates space for support, accountability, and connection in a safe, professionally guided setting.

Is IOP the Right Fit?

If you are wondering whether IOP is the right fit, the answer depends on your symptoms, your safety, your support system, and how much structure you need right now. Some people begin in IOP. Others step down into IOP after completing a more intensive program.

To better understand that next level of care, read What Is Partial Hospitalization (PHP)? If your concerns are more related to alcohol or drug use, What Qualifies as Substance Abuse? and Do I Have a Drinking Problem? may help you recognize when it is time to seek support.

Taking the First Step Toward Support

Reaching out for help does not mean something is wrong with you. It means you are paying attention to what you need. For many people, IOP is where healing becomes more manageable, more consistent, and more possible.

In short, our Adolescent IOP in West Bloomfield is a structured outpatient mental health program designed specifically for teens.
The Top Docs list, as published by Hour Detroit, is not just a reflection of credentials, it’s about trust. These awards are based on nominations from physicians who recognize exceptional care in their colleagues. After nominations are collected, PRS verifies credentials and reviews any infractions through applicable boards and rating agencies before finalizing the list. Hour Detroit itself plays no role in the selection process.

To be recognized by peers in such a rigorous process speaks volumes about Dr. Maroof’s professionalism, clinical excellence, and compassionate presence.

At River’s Bend, we believe that healing is deeply personal—and that recovery is best supported by teams who care beyond the diagnosis. Dr. Maroof exemplifies this belief, and we are honored to have her contribute to the recovery journeys of our clients.

Congratulations again to Dr. Maroof on being named a 2025 Top Doc. We celebrate you—not just for this accolade, but for the steady, thoughtful care you provide each week.

A Story of Strength, Family, and the Lifelong Impact of Compassionate Mental Health Care

When Lori first walked through the doors of River’s Bend, she wasn’t there for herself. She was a mother, fighting for her 12-year-old son, searching for answers after years of misdiagnoses, behavioral challenges, and growing substance use issues. What she found instead was a place that would not only help her son, it would help her rediscover herself.

Now in her sixties, a proud grandmother and a retired dog groomer, Lori is still an active client at River’s Bend. And she’s never been more confident in saying: “I think I’ll be in therapy forever… and every human could benefit from it.”

Reaching Out: “We Just Couldn’t Handle It on Our Own Anymore”

Lori’s journey with River’s Bend began nearly two decades ago. At the time, her family was in crisis.

“We had been through so many doctors,” she shared. “My son was misdiagnosed with ADD, and we couldn’t seem to get any clear answers. He was getting into trouble, starting to experiment with substances, and we were out of options. That’s when we found River’s Bend, and Stephanie.”

Stephanie Howey, a licensed therapist and Co-owner at River’s Bend, has become more than a clinician in Lori’s life. She’s become what Lori calls her “guardian angel.”

“Stephanie started working with my son when he was 12. He’s now 31. Since then, she’s worked with my daughter, my late husband, and now me. She’s part of our family.”

The Power of Being Heard: What Makes Therapy at River’s Bend Different

Lori’s transformation began when she realized therapy at River’s Bend wasn’t just about diagnosis or treatment; it was about being seen.

“She never tells you what to do,” Lori says about Stephanie. “Instead, she helps you get there on your own. She asks: ‘How does that make you feel? What do you think is the right next step?’ And before you know it, you’re figuring things out yourself. That’s powerful.”

Having experienced other therapists before, Lori knew what it felt like to be dismissed.

“I once told a different therapist I wanted to save my marriage. Her answer was just, ‘Absolutely not.’ She didn’t even know me. But I know Stephanie would’ve said, ‘That depends. What are you feeling? What are your priorities?’ It’s a different level of care.”

Grief, Growth, and the Importance of Boundaries

Five years ago, Lori lost her husband. It was a devastating moment, but one that deepened her relationship with her therapist and with herself.

“Stephanie walked me through my grief. She helped me set boundaries, especially with my son, and reminded me that protecting my peace wasn’t selfish. It was necessary.”

This past spring, Lori experienced a turning point. “I had to draw a hard line with someone I love deeply. It was painful. But afterward, I felt stronger. Stephanie was so excited for me, cheering me on. She said, ‘Yes! You’re getting it!’ That moment was a breakthrough. I finally realized it was time to start living for me.”

Therapy Isn’t Linear, It Is Lifesaving

Like many people, Lori once believed that healing would follow a straight line. But over time, she learned that therapy is a winding path filled with “lightbulb moments.”

“I can be crawling out of my skin with anxiety,” she shares, “and just one session with Stephanie, one conversation, brings me back. Every time I leave, there’s a shift. A deeper understanding. A new tool in my belt.”

What sets River’s Bend apart, Lori explains, is the level of dedication she’s experienced over the years.

“There’ve been times I’ve texted Stephanie at 1:00 AM. And she responds. Maybe not immediately, but she always shows up. She’ll squeeze me in, call me after hours, or move things around to make space. You don’t find that kind of support just anywhere.”

Therapy as a Lifeline and a Legacy

For Lori, therapy at River’s Bend isn’t just something she does for herself. It’s become part of her family’s story.

“When my daughter struggled after college, I told her, ‘Go see Stephanie. Just once.’ She was skeptical, but she went. That was 15 years ago, and she’s still going.”

Lori’s voice trembles with emotion as she reflects on what River’s Bend has given her family: “I can’t put into words what Stephanie means to me. I’d cry trying. She’s changed our lives. Truly.”

“Every Human Could Benefit from Therapy”: Lori’s Message to Others

When asked what she’d say to someone hesitant to reach out for help, Lori doesn’t hesitate.

“There’s still a stigma about therapy, but it’s getting better. People need to understand, it’s not just for when you’re in crisis. Sometimes, you just need someone outside your family or friend group. Someone who isn’t biased. Someone who can help you see the hard things clearly.”

She adds, “Therapists are professionals for a reason. They help with grief, addiction, anxiety, anything you’re carrying. And therapy doesn’t mean something’s wrong with you. It means you’re strong enough to ask for help.”

Why River’s Bend? A Community of Care That Goes Beyond the Session

Lori’s story reflects what many clients say about River’s Bend: that it’s not just a clinic. It’s a place where you feel safe, validated, and genuinely cared for.

As Lori puts it, “They’ve stepped up for me again and again. Over the years, everyone at River’s Bend has been wonderful. I’m indebted to them for life.”

Whether you’re struggling with anxiety, navigating a family crisis, healing from substance use, or grieving a loss, River’s Bend offers integrated, evidence-based care in a setting that feels more like family than a clinic.

Ending the Stigma, One Story at a Time

Lori’s story is a testament to the power of therapy, especially when it’s delivered with empathy and skill. It’s a reminder that healing isn’t just possible, it’s personal.

If you’ve been unsure about reaching out for mental health or substance use support, let Lori’s words be your encouragement:

“Just go once. That’s all. You don’t even have to talk if you don’t want to. But that one time… it might just change your life.”

Ready to Start Your Own Journey?

River’s Bend is a nationally accredited, private outpatient mental health and substance use clinic with locations in Troy and West Bloomfield. Virtual sessions available.

Honoring a Life Lost and the Call to Support Mental Health

At River’s Bend PC, we are heartbroken by the recent news of the young football player who tragically took his own life at just 24 years old. His story has deeply moved our entire team and serves as a painful reminder that even those who appear strong, successful, and surrounded by support may be carrying invisible pain beneath the surface.

Mental health challenges do not discriminate. Grief, depression, trauma, and hopelessness can affect anyone, regardless of age, achievement, or circumstance. Reports shared that the young athlete had been struggling with his mental health following the loss of his mother, a grief no one should have to face alone. Our hearts go out to his family, friends, teammates, and the entire community mourning his loss.

The Silent Weight of Grief and Depression

For many, grief is not a single event, it’s an uneven journey that can lead to deep emotional pain. The loss of a parent, loved one, or mentor can trigger sadness, guilt, anger, or disconnection. When these emotions go unspoken, they may develop into anxiety, depression, or suicidal thoughts.

In athletics and other high-performance environments, the pressure to “stay strong” can make it even harder to ask for help. Yet mental pain requires care, not endurance. True strength is found in reaching out, not staying silent.

Breaking the Stigma: Compassionate Mental Health Care in Michigan

At River’s Bend, we believe healing begins when people feel seen, heard, and supported without judgment. Our behavioral health clinicians in Troy and West Bloomfield work with adults, young adults, and families to:

Build resilience after loss or trauma

Strengthen emotional regulation and coping tools

Develop healthier ways to process pain and change

Restore hope and confidence in the future

We use evidence-based therapies such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care to help clients address both the symptoms and root causes of emotional distress.

Our mission has never been more urgent: to ensure that no one feels they must face mental health struggles alone.

Recognizing the Warning Signs of Suicide

Suicide prevention begins with awareness. Anyone can experience suicidal thoughts, regardless of background, age, or success. Recognizing the warning signs can save lives.

Common signs include:

Talking about wanting to die or feeling hopeless

Withdrawal from friends, family, or favorite activities

Increased anxiety, anger, or substance use

Giving away belongings or saying goodbye

Extreme mood changes, loss of interest, or emotional numbness

Not everyone shows visible symptoms before a suicide attempt. Pain is often hidden behind achievement, humor, or calmness. Regular, honest check-ins with loved ones can make a life-saving difference.

If someone seems “off,” ask how they’re really doing, and listen without judgment.

If You’re Struggling, You Are Not Alone

If you or someone you love is struggling with depression, grief, trauma, or suicidal thoughts, help is available right now.

Call or text 988 to reach the Suicide and Crisis Lifeline free, confidential, and available 24/7.

For those in Michigan, River’s Bend PC offers compassionate, accessible mental health treatment and recovery services, including:

Intensive Outpatient Programs (IOP) for mental health and substance use

Partial Hospitalization Programs (PHP) for structured daily care

Individual and group therapy for adults and adolescents

Our team works collaboratively to build personalized treatment plans that support long-term healing, safety, and stability.

Healing Happens Here

The loss of this young athlete reminds us of a powerful truth: success and strength do not make someone immune to pain. Mental health challenges can affect anyone, but compassion, connection, and professional care can help turn suffering into healing.

At River’s Bend PC, we remain steadfast in our mission to raise awareness, offer hope, and walk beside those who are hurting.

You are not alone. Healing happens here.
